Core Solutions Group LLC is searching for a dedicated Case Manager to join our team in delivering top-notch medical and dental readiness support to the Minnesota Army National Guard. As a key player in our non-clinical team, you'll leverage your technical expertise to aid medical staff in the identification, evaluation, resource allocation, monitoring, and disposition determinations for service members in need of support for health maintenance, illness, injury, or disability management. Your case management skills will be essential in a range of domains including assessment, planning, implementation, coordination, and the monitoring and evaluation of service options to meet the Army National Guard (ARNG) Soldiers' health and human services needs. This full-time role is based on-site from Monday to Friday, ******** , at the Cottage Grove Armory.
Please note: This position is contingent upon contract award. Responsibilities: Effectively maintain soldier medical and dental records, adhering to Army Regulation (AR) 40-66 guidelines. Secure and label files, output products, and storage media as per security classification. Oversee the creation, maintenance, and processing of medical and dental records for members at all stages of service. Ensure the completeness of medical records before key events such as Basic Military Training and Medical Readiness Events (MREs), facilitating deployment eligibility. Perform regular medical/dental record inventories. Digitize and categorize medical documents. Generate and distribute new system reports with DOD System software updates. Facilitate service members in setting up medical and dental appointments. Engage in monthly eCase reviews and coordinate treatment plans as necessary. Conduct interviews, ensure effective communication, and maintain documentation within the eCase module. Utilize MEDCHART applications for charting and tracking Soldier care. Oversee the continuum of care from condition identification to return-to-duty or administrative separation. Educate soldiers on medical and dental readiness resources and lifestyle modifications. Update health records, coordinate with the DSS on complex cases, and prepare medical documentation for investigations as required. Compile and provide medical and dental documents for case management to Medical Command. Analyze electronic data for early identification of non-deployable soldiers. Maintain active case management to ensure effective tracking and communication. Adhere to Individual Medical Readiness (IMR) and deployment regulations. Maintain HIPAA standards and complete annual certification. Participate in MRE and SRP events, providing required services. Qualifications: A Registered Nurse (RN), Licensed Vocational/Practical Nurse (LVN/LPN), Social Worker, Army MOS of 68 or 42 series level 4 or similar credentials with a relevant degree, or a Care Coordinator with six years' experience. A degree from an accredited college or university, preferably with case management experience or a minimum of five years in healthcare. Active and valid HIPAA certification. MUST BE ABLE TO PASS A GOVERNMENT LEVEL CRIMINAL BACKGROUND CHECK. Strong organizational and communication skills. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook). Ability to work independently and as part of a team. Flexibility to work during normal duty hours, weekends, and events as required. Benefits: We offer a comprehensive benefits package.
