Description The Behavioral Health area in the Human Services and Public Health (HSPH) department is seeking two Senior Psychiatric Social Workers for the HART Case Management team. These positions provide specialized mental health targeted case management services to adults with substance use disorders, lower intellectual or cognitive functioning, and/or other challenges and risks to self and complex psychiatric and behavioral disorders who have been found incompetent by the criminal court and are civilly committed to state hospitals.
Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500 referral bonus. For more information visit employee referral program. Location and hours: These positions are hybrid and will require travel throughout Hennepin County, client residence, treatment facilities, hospitals, and other locations throughout the state of Minnesota as needed. Transporting clients is sometimes required. Work hours are Monday through Friday, 8 a.m. to 4:30 p.m. with some flexibility. While these positions are designated as hybrid, based on current requirements hires must reside in or be willing to relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will: Provide targeted case management services to clients by monitoring, coordinating, assessing, referring, and planning services and resources. Perform person-centered clinical assessments to determine treatment goals and intervention plans based on the individual's identified needs. Build collaborative relationships with clients and business partners. Assess clients' readiness for safe provisional discharge from regional treatment centers and work collaboratively with providers. Monitor provisions of the commitment order; including providing reports to the court regarding clients' progress and testifying at legal hearings. Supervise community-based clients; coordinate all aspects of clients' provisional discharge order including psychiatric aftercare, medication management, housing, vocational development, financial assistance, and ongoing treatment needs. Refer clients to culturally appropriate community services and work effectively in multidisciplinary teams. Make field visits to clients' homes or community residential setting. formulate person-centered treatment planning that is culturally informed. Prepare written commitment reports including revocation requests. Need to have: Master's degree or higher in social work or closely related field and two years or more of post-master's degree experience in a psychiatric or mental health setting. Current licensure as an Independent Clinical Social Worker (LICSW) by the Minnesota Board of Social Work or as a Professional Clinical Counselor (LPCC) or Marriage and Family Therapist (LMFT) by the Minnesota Board of Marriage and Family Therapy. Qualification as a Mental Health Professional in the state of Minnesota. A valid driver's license, the ability to obtain a Hennepin County driver permit, and daily access to reliable transportation sufficient to transport clients and to travel to a variety of Hennepin County community locations. Nice to have: Experience: Working with individuals living with serious and persistent mental illness and/or substance use. Providing targeted case management. Conducting clinical assessments. Working with involuntary clients. Knowledge of: Person-centered practice in relation to working with involuntary clients. County and community resources. Cultural differences in beliefs and practices relating to mental health, treatment, family structure, and dynamics. Social work best practices and ethics. Civil commitment. Ability to: Work collaboratively as part of a team. Contribute to an environment that recognizes and respects cultural, lifestyle and socio-economic differences. Manage high-risk clients in the community. Communicate effectively to multiple interested parties using excellent verbal and written communication skills. Candidates with fluency in another language(s) in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
