Responsibilities
• Conduct psychosocial assessments for residents and update service plans• Document and enter data in client charts and DPH reporting system• Communicate with primary care teams to ensure adherence to appointments and medications• Provide appointment reminders, health education, and treatment adherence support• Assess and provide counseling, advocacy, and referrals to community partners• Coordinate additional services needed, such as IHSS and financial management• Assist with housing navigation and support moving process• Lead education and training sessions on clinical and psychosocial issues• Plan community activities for residents• Participate in staff training and development activities
Requirements
• 3+ years of experience in social work, counseling, or similar field• Licensed clinical degree preferred• Experience working with individuals living with HIV/AIDS, mental illness, homelessness, and substance use• Ability to hold boundaries and set limits with challenging behaviors• Bilingual in Spanish preferred• Experience with crisis intervention, conflict resolution, and de-escalation• Experience with public benefits or housing assistance• Strong written and oral communication skills• Ability to work independently and as part of a team• Proficiency with basic computer programs• Understanding of intersectionality and needs of communities impacted by homelessness• Strong commitment to housing justice and inclusive programming
