Responsibilities
• Assist with activities of daily living according to unique care plans• Accompany clients on recreation and leisure activities• Assist in behavioral and personal care services• Implement individual plans as written• Increase independence in the community• Ability to understand and implement a variety of Community Support Plans and Individual Service Plans per client outcomes and goals• Ability to track and chart data daily in line with company expectations• Attend, assist, and engage in team meetings as needed• Maintain communication with managers and supervisors• Complete assigned trainings within required time
Requirements
• Experience working with individuals of varying needs and diagnoses• CPR and First Aid certification• At least 18 years of age• English proficiency to ensure seamless communication with individuals
