T he Triage Nurse Coordinator is a vital healthcare team member responsible for providing timely and accurate triage services for patients received on call or seen and evaluated in the Emergency Department, Urgent Care, or another acute medical facility. This role involves assessing patient needs, assisting physicians, and ensuring efficient communication between patients, medical staff, and other healthcare providers. The Triage Nurse coordinator functions in the walk-in clinic and assists with assessing and triaging incoming patients. The Triage Nurse Coordinator upholds the mission and values of OCI by delivering compassionate care and promoting patient well-being.
This position will work a rotating schedule that will include alternating Saturdays (8:00 AM to 12:00PM). Monday - Friday 8:00am to 5:00pm CORE RESPONSIBILITIES Conduct comprehensive assessments of patients' medical records, test results, treatment plans, and present medical concerns. Utilize triage protocols and clinical judgment to prioritize patient care based on the severity of symptoms. Triaging, screening, and returning patient and provider phone calls, gathering relevant medical information, and documenting details accurately. Review charts, notify physicians of patients' progress and lab results completed by referring physicians, recognize abnormalities, and take appropriate action per physician instructions or set guidelines. Guide, educate, and instruct patients regarding self-care, medication administration, and follow-up care. Coordinate patient appointments, referrals, and transfers to appropriate healthcare facilities as needed. Assures proper insurance referral/authorization information is obtained and provided to the insurance verification department. Remain flexible and responsive when patient activity and workload changes occur . Follows all safety rules in the workplace, complies with OSHA guidelines, and assures that medical equipment is in proper working order and readiness. Maintain patient confidentiality and HIPAA Perform other duties as assigned SKILLS/KNOWLEDGE and EXPERIENCE Current unrestricted LPN/RN certification in state of CMA considered with experience in 2-3 years recent acute care hospital, triage nurse or emergency care Orthopedic experience Ability to work efficiently in a fast-paced environment. Excellent interpersonal and customer service Strong clinical assessment and critical thinking skills with a flexible and creative Excellent oral and written communication skills; able to communicate professionally and effectively with all members of the healthcare team and in simple, concise, and soothing language to patients and their families.
