This position has an annual salary of between $50-$65K plus a very generous benefits package including: medical, dental, and vision plans; generous paid time off, 403B retirement plan with employer match, tuition reimbursement and other benefits.The Treatment Coordinator/QIDP is responsible for coordinating the training and habilitation program of the assigned residents with residential staff, educational and vocational rehabilitation programs, and other needed services.ESSENTIAL RESPONSIBILITIES§ Serving as member of interdisciplinary team when needed, in order to coordinate integration of various services in planning, implementation, and evaluation of an individual's treatment plan.§ Generating Needs/Capabilities list annually, with updates as needed.§ Ensuring that each assigned client has (1) an activity schedule which gives an hourly account of the resident's day, 24 hours per day, 7 days per week, (2) ongoing continuous active treatment, and (3) implementation of an activity schedule as documented.§ Acting as resident advocate in areas that relate to resident cleanliness, ensuring adequate clothing for individuals, personalized and clean bedrooms, toiletries, etc.§ Completing reviews of each assigned resident's individual treatment plan by summarizing progress or need for program modification as supported by concrete, measurable data obtained by direct care staff.§ Acting as liaison with day/workshop programs, with ongoing face to face and phone contact with day providers and ensuring coordination of services between day and residential components.§ Ensuring that IPP's and day program progress notes are received in timely fashion and remain on file.§ Ensuring that direct care/clinical staff are trained in behavioral interventions and active treatment areas, so that all staff working with individuals are able to carry out treatment plans as written.§ Coordinating Annual Review process by:Ensuring reports from all disciplines are collected in timely mannerPreparing annual report and Functional Assessment and/or update on Developmental History, Medical, Nutrition, Recreation, Sensorimotor Development, Communication, Day program/Workshop, Skills Assessment, Behavior Socialization/Incident Reports, Review of Previous Treatment and Progress, Financial Status, etc.Documenting rationale for type of intervention upon needs addressed in upcoming year; and,Generating goals, objectives, and interventions for indicated needs.Qualifications§ Bachelor's Degree from an accredited college/university in human services-related field.§ Must possess leadership abilities and be able to coordinate a team of clinical and management staff.§ Must have excellent organizational skills and good writing ability.§ Must possess the ability to deal tactfully, professionally, and effectively with the residents, personnel, family members, visitors, government agencies, and the general public.§ Must have a good working knowledge of direct care responsibilities and can perform such duties.§ Must have a basic knowledge of all clinical areas that would comprise the interdisciplinary team for individuals with intellectual disabilities.§ Must be willing to work in multiple locations and be willing work flexible hours.AHRC New York City is an Equal Opportunity Employer.
