Job summary
Jewish Family and Children's Service's (JFCS) is seeking a Treatment Coordinator for the West Valley Behavioral Health Center.The Treatment Coordinator provides ongoing care-coordination services to address the client and family's needs.The position involves coordinating services, identifying resources, and resolving barriers.
Job seniority: mid-to-senior level
Responsibilities
• Assist clients and families in obtaining community support and resources.• Perform ongoing coordination of services in accordance with the treatment plan.• Respond to initial crisis calls and identify necessary resources.• Engage formal and informal resources for the client.• Coordinate behavioral-health services with other agencies.• Demonstrate a positive, strengths-based attitude of teamwork and shared leadership.
Requirements
• Bachelor's Degree in behavioral health field preferred.• Relevant experience working in a behavioral-health setting.• Level 1 Fingerprint Clearance Card.• Verification of education, employment, references.• MVR matching JFCS driving guidelines.• AZ Driver's License and Valid Auto Insurance.
