Description Location: UCHealth Anschutz Outpatient Pavilion, US:CO:Aurora
Department: Transplant Center
FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)
Shift: Days 
Pay: $40.86 - $63.33 / hour. Pay is dependent on applicant's relevant experience
Summary: Provides care across the continuum of care for post-transplant. Collaborates with multi-disciplinary team to enhance patient care. Acts as both clinician and educator. Serves as a liaison to regulatory agencies (CMS, UNOS, etc.)
Responsibilities:
Coordinates transplant team activities with patients, families, organ donor networks, regulatory agencies, and transplant networks and agencies. Coordinates care of transplant patients through collaboration with attending surgeons, hepatologists and staff. Coordinates plan of care with the transplant clinic and the financial coordinator. Documents per policies, procedures and regulatory requirements. Participates in notification plans for transplant teams. Managing, monitoring and triaging the care of pre and/or post-liver transplant patients. Assists in maintaining records of the evaluation information on transplant patients. Education of the transplant process to patients and family/friends. May serve in on-call capacity. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. Bilingual Spanish speaking candidate would be ideal. Requirements: Minimum Required Education: Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience. Preferred: Bachelor's degree in Nursing from an accredited nursing program. Required Licensure/Certification: State licensure as a Registered Nurse (RN). Basic Life Support (BLS) Healthcare Provider and any relevant certification as determined at position level. Minimum Experience: 2 years of experience in a related field of specialty. Preferred: Transplant coordination experience. Clinical care of transplant recipients. BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire.
