Transition Case Manager For - School Year

TRENTON SPECIAL SCHOOL DISTRICT JOB DESCRIPTION Position Title: Transition Case Manager for ******** School Year Term of Employment: Two Hundred (200) Days Contract Immediate Supervisor: Building Level Principal and Special Populations Supervisor Qualifications: Bachelor of Science/Bachelor of Arts in Education, Human Services or a related field and two (2) years of teaching experience or providing services to individuals with disabilities. Must have a warm and friendly personality, be sensitive to the feelings and needs of others, be able to relate well to students, and be willing to fulfill responsibilities in accordance with the Trenton Special School District policies and philosophy.
The Transition Case Manager is responsible for managing the Transition School to Work Program for Peabody High School. The role of the Transition Case Manager may require additional roles and/or responsibilities. The Transition Case Manager position is contingent upon grant funding.
