Responsibilities
• Provide risk assessments, evaluations, safety plans, treatment plans, and recommendations for treatment.• Work collaboratively with an interdisciplinary treatment team.• Complete ASAM dimension criteria as part of the evaluation process.• Experience and knowledge of completing and executing a pink sheet.
Requirements
• Master of Social Work degree from an accredited university.• Preference given to candidates licensed as LCSW in Utah.• Under-filling options available for CSW, ACMHC, and AMFT.• Pass pre-employment criminal background check and drug screening.• Proficiency in computer-related skills.
