Telephonic Care Manager Rn

Compensation:
Hourly rate: $37 - $40, commensurate with experience.
Benefits available for full-time employees after 90 days.
Eligibility for a 401(k) with company match after one year of service.
Qualifications:
Registered Nurse with active Oregon license.
Prior experience as a Care Manager, preferably in a telephonic setting within healthcare.
Proficient in computer skills, including documentation.
Comfortable with a 90-day+ contract.
Familiarity with Medicare/Medicaid/Managed care plans is advantageous.
Responsibilities:
Conduct thorough assessments of members' healthcare needs.
Develop personalized care plans based on members' needs and preferences.
Provide comprehensive member education.
Engage in outbound and inbound calls, with a focus on member outreach.
Accurate data entry and record-keeping.
Proficiency in managing multiple computer systems.
Strong time management skills to prioritize tasks effectively.
