Social Worker 2 (SW2): $24.75 - $34.65 per hour Social Worker 3 (SW3): $29.41 - $41.16 per hour

We are seeking a dedicated Social Worker (MSW) to provide non-clinical biopsychosocial assessments and interventions for our inpatient floors and outpatient emergency department. The MSW will perform crisis intervention, patient and family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other resource needs. This role involves working with intensive care units, Trauma, Oncology, NICU, Transplant, and ED.
Key Responsibilities
Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs. Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual barriers. Maintain thorough knowledge of Medicare, Medicaid, and private payer regulations and processes, and understand managed-care concepts and length-of-stay management. Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. Coordinate and facilitate meetings between the care team and patients/families/caregivers, assisting in making difficult decisions while honoring their preferences and values. Provide emotional support to patients and family members during complex issues such as end-of-life, terminal illness notifications, death notifications (with physician), and crisis interventions. Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. Qualifications
Education: Must have a Master's Degree in Social Work from an accredited school. Licensure: State license eligible. Experience: Minimum of 1 year of hospital Social Work experience preferred. Additional Information
Positions Available: Social Worker 2 (SW2): Must have LMSW; RPC will offer clinical supervision. Social Worker 3 (SW3): Must have LCSW license.
