Provides Social Work services to facilitate the rehabilitation process, the achievement of an optimal level of health care and the patient's/family's adjustment to the illness/disability.
Works closely with the healthcare team to formulate and implement early discharge planning based on assessed medical, emotional and financial needs, as appropriate for age, integrating the comprehensive psychosocial assessment findings. Arranges placement/return to extended care facilities or reintegration into the home/ community. Assesses biopsychosocial factors affecting health, adjustment, patient/family functioning or care plan, appropriate to growth and development needs of specified age groups. Mobilizes and utilizes family/community resources to meet identified needs. Serves as liaison and patient advocate in coordination and procurement of services, addressing conflict resolution, Power of Attorney issues, ethical concerns and liaison with various agencies (Department of SSI, Medicare, League of Older Americans, etc.) Meets documentation standards for quality, timeliness and confidentiality as defined by organizational/departmental policy. Enhances professional growth by participating in activities which foster social work identity growth and development. Performs discharge planning on patients meeting criteria and/or upon referral. Coordinates home care referrals (except for IV therapy). Performs financial counseling as appropriate including, self pay, Medicaid, Medicare and charity care payment options. Assist with transfer (payer approvals, transportation, etc.) Arranges transportation with support from Resource Center. Serves as the coordinator for agency/staff and legal processor involving abuse and neglect of patients. Acts on behalf o the patient as an advocate and facilitator in application for Guardianships. Participates in departmental evaluation and development on an ongoing basis. Promotes Carilion Clinic Culture of Excellence and quality service through teamwork, respectful communication, maintenance of a professional environment, partnership with those we serve, and anticipation of patient/customer needs. Responsible for department staffing coverage/scheduling. Acts as proctor for student interns. Assists in training and orientation of new social work employees. Submits monthly reports to Manager as assigned. What We Require: Education: Bachelor's Degree in social work or Bachelor's Degree in related field with comparable experience required. Master Degree preferred. Experience: Minimum two years social work experience in health care facility preferred. Other Minimum Qualifications: Demonstrates knowledge of Social Work theory and practice; proven expertise as a social worker; ability to handle crises effectively and problem solve successfully. Satisfactory completion of orientation; positive interpersonal oral communication skills; effective written communication skills; analytic skills; integrity; innovation; team player; courteous; ability/willingness to work as an integral member of a multi skilled team.
