Job summary
Support weekend operations for Circle of Life, a leading hospice provider in Northwest ArkansasResponsible for developing and overseeing care plans for patients' psychosocial, financial, and emotional needsLiaise with community resources and provide bereavement support as needed

Responsibilities
• Develop and oversee care plans for assigned patients' psychosocial, financial, and emotional needs• Provide coverage for inpatient and/or outpatient Hospice program as needed• Liaise with community resources to find support for patients and families• Flexibility with schedule to accommodate patient and organization needs• Assist with mentoring interns and other students• Facilitate bereavement programs as needed
Requirements
• Current Arkansas Social Work license without restriction• Emotionally mature, objective, sensitive to people and their problems• Excellent computer, phone, reading, writing, and communication skills• Ability to organize information, time, and workload efficiently• Ability to work outside normal office hours and handle emergencies• Good working knowledge of Social Work scope of practice and regulations• Satisfactory results of current TB skin test, annual TB fit test, and annual flu vaccine• Knowledge of dynamics of death and dying, bereavement and end of life issues preferred• Experience in a hospice or healthcare environment preferred
