Primary Purpose of Organizational Unit: The purpose of the Department of Exercise and Sport Science (EXSS) is to discover, create and promote knowledge of human movement to improve the quality of life of individuals and society. This will be accomplished by establishing and maintaining well-funded research programs with a high-quality, productive research faculty who are committed to the research mission of UNC Chapel Hill and EXSS. The department provides leadership in teaching, research, and scholarship in the discipline of physical education, exercise, and sport science. A major commitment of the department is to discover, interpret, disseminate, and apply discipline specific knowledge to the university, state, national, and international communities. The department pursues excellence in educational programs including physical activity instruction, undergraduate academic and graduate academic programs. The mission of EXSS is further enhanced through interdisciplinary cooperation and departmental faculty securing competitive extramural funding in the form of contracts and grants. These endeavors enable the department to advance knowledge and thus serve the state and nation.


The Matthew Gfeller Center and the Center for the Study of Retired Athletes are housed within the Department of Exercise and Sport Science. The mission of the Matthew Gfeller Center is to improve the prevention, evaluation, management, and rehabilitation of sport-related traumatic brain injuries through research, education, and clinical practice. The Matthew Gfeller Center is a diverse team exceeding 25 individuals. These core faculty, staff, and trainees are supported by extramural research grants and contracts totaling approximately $10M. In addition, the Matthew Gfeller Center secured a $12.5M gift to develop the Transforming Health and Resilience in Veterans (THRIVE) Program.
Position Summary: This position will primarily serve as a Lead Social Worker for the Transforming Health & Resilience in Veterans (THRIVE) Program. The THRIVE Program is a community-based clinical outreach program designed to provide care to Veterans and first responders who are experiencing the effects of traumatic brain injuries (TBI) or post-traumatic stress (PTS) resulting from a career serving our nation. The three-phase program will 1) evaluate and diagnose physical, cognitive, and other health conditions during a 3-day evaluation period, 2) treat these diagnoses during a 3-week intensive outpatient program, and 3) continue to follow up with the Veteran or first responder for up to 12 months following program completion. In its full capacity, the program will see approximately 150 Veterans per year and will provide a referral network and post-program follow-ups for as many as 400 Veterans and family members every year.
This position will work within a comprehensive interdisciplinary medical team to assess patient needs. The position will provide screening interviews to identify at risk social and behavioral patients entering the THRIVE Program. The position will coordinate referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patient. When identifying at risk patients, this position must be able to make critical decisions regarding the patients' mental/physical health needs to include enhanced social work assistance.
This position will serve as the lead for the Social Work team and work under minimal supervision to:
Social Work:
- Assist patients with navigating appointments through the THRIVE Program 3 day evaluation and Intensive Outpatient Program
- Build relationships with patients that foster early communication of risky situations to mitigate the need for crisis management
- Prepare clients' discharge note with clinical team and reviews post-discharge plan requirements
- Assesses patients' needs and provides proper clinic and community referrals
- Advocate on behalf of patients to ensure we are meeting their needs
Treatment/ Counseling:
- Assess client needs and intervene appropriately
- Use counseling techniques to teach appropriate behaviors and develop behavioral programs
- Follow up with patients for 1 year post discharge. Communicate clinically significant information to providers
- Maintain contact with relevant providers to monitor progress and coordinate treatment efforts
Records Management:
- Maintain clients' records by reviewing case notes; logging events and progress
- Complete required documentation and electronic data entry to assist with continuity of care, program evaluation, and outcome data.
- Participate in regularly scheduled clinical supervision, case consultations, case management and team meetings.
Communications/Program Administration:
- Serve as liaison between the THRIVE Program patient and the referral source(s)
- Demonstrate the ability to provide appropriate care to Veterans, first responders, family members and caregivers
- Represent the university and THRIVE Program team in a professional manner.
- Implement new processes and procedures
- Oversee and manage Spousal Support Group
- Train and onboard new case managers and program staff Minimum Education and Experience Requirements: Bachelor's degree in a discipline related to the field assigned and one year of related training or experience; or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.

Preferred Qualifications, Competencies, and Experience: - Professional experience with direct patient care with mild to moderate Traumatic Brain Injuries
- Experience working with active duty and/or veterans

- Ability to maintain a positive and cooperative attitude
- Ability to organize and prioritize with minimal direction
- Ability to work under moderate to high measures of stress

Position/Schedule Requirements: Clinic – UNCH or ACC, Evening work occasionally
