The Long-Term Services and Supports (LTSS) area of Human Services Public Health (HSPH) is seeking two Social Work Unit Supervisors to join the Case Management team. These positions supervise a team of Social Workers and Case Management Assistants who provide services to people who receive Developmental Disabilities (DD) services and Long Term Care (LTC) waiver case management. The programs administered serve people with developmental disabilities and related conditions and include people with intellectual and developmental disabilities, brain injury, mental health, complex medical, and physical disabilities. The federal and state programs that serve these populations includes: Community Access for Disability Inclusion (CADI) waiver; Brain Injury (BI) waiver, Community Alternative Care (CAC) waiver, Developmental Disabilities (DD) waiver, and/or Personal Care Assistance (PCA).

 
The mission of LTSS is to collaborate with communities and engage seniors and people living with disabilities to identify their strengths and needs. We offer federal waivers, state plan and community-based programs to support a person's overall health and well-being.

 
Location and hours:
These positions are remote. Remote workers may be required to come on-site 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practical. Based on business needs of this position, hires must live in or relocate to Minnesota. About these position types: These are full-time, benefit-earning positions. You must be a current regular or probationary employee of Human Services Public Health to apply. Limited duration employees are not eligible.

In these positions, you will: Establish and maintain standards for customer service and quality work through hiring staff, coaching, training, and evaluating performance. Work with staff to provide consultation, information and referral, outreach, assessment, crisis intervention, service coordination and case management. Manage successful change by providing leadership on county vision and mission, involving team in problem solving, and promoting thoughtful communication. Develop, recommend, and implement policies and procedures ensuring compliance with human resource rules, state statute, and assessment and case management best practice and standards. Work with the team to develop team goals, performance results and plan for implementation. Lead the team in monitoring adherence of case management standards as well as county and state standards, processes, and procedures.  Serve as program lead, subject matter expert, and ensure program compliance with applicable state and federal regulations. Facilitate continuous process improvement of systems and programs by determining needs and improvements, developing plans, developing policies, procedures, and evaluating outcomes for services. Promote and maintain a work environment where all customers are respected and treated respectfully. Need to have: One of the following: Master's degree or higher in social work or closely related field (or licensed as a graduate social worker by the Minnesota Board of Social Work) and three years or more of experience as a social worker. Bachelor's degree in social work, psychology, sociology, or human services, and 27 quarter credits or more of graduate work in areas such as social work methods, human growth and behavior, principles of social welfare, and four years or more of supervised experience as a social worker. Six years or more of experience as a Senior/Psychiatric/Child Protection Social Worker at Hennepin County. Nice to have: Experience: One or more years or supervisory, management, and/or leadership experience in a related position or field Serving diverse populations Knowledge of: Assessment, planning, and service development for people with developmental disabilities and related conditions, brain injury, mental health, complex medical, and physical disabilities County and community resources Social work and case management principles, best practices, and ethics Supervisory principles and practices Physical and mental illnesses and their impact on personality and behavior Person-centered thinking and practices Ability to:  Foster an environment that recognizes and respects cultural, lifestyle, and socioeconomic differences Establish and nurture effective working relationships with employees, other professionals, agencies, and residents Demonstrate the supervisor competencies Analyze problems and develop and implement effective courses of action Read, interpret, and develop policies and procedures Communicate effectively using verbal and written methods About the department: Human Services delivers a variety of services to individuals or families that assist with basic needs or encourage client change around specific objectives.
