We are currently seeking an experienced Social Services Assistant to join our mission of bringing quality care to Phoenix, Arizona. Position: Social Services Assistant The Social services Assistant will assist the Director of Social Services in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures. Job Type: Full Time, Monday - Friday
Pay Rate: Starting at $50,000/year

Ability to build 1:1 relationships with residents and families as needed
Actively oversee the coordination of care for patients including the outsourced provider, physicians, family members, and or other outside agencies
Knowledgeable of starting guardianship process
Knowledgeable of steps needed for annual guardianship reports
Knowledgeable of AHCCCS application filing process for LTC insurance
Knowledgeable of interacting with behavioral residents
LTC experience strongly preferred Desert Peak Care Center 8825 S 7th St.
