ABOUT LAUREL HILL CENTER Established in 1972, we are committed to helping people with psychiatric disabilities make choices and acquire skills that increase their self-reliance and ability to live and work in the community. Using behavioral health evidence-based practices, we effectively address several social determinants of health, such as housing, employment, and education. Our efforts are focused on supporting those in our community who have been diagnosed with a serious mental illness and require intensive community support. We provide housing resources such as rental subsidies, housing navigation services, and permanent supportive housing (PSH). A DAY IN THE LIFE OF A CASE MANAGER In this case management position, you provide social services to individuals who struggle to maintain their residences. Following best practices, you focus on keeping individuals in a stable housing situation while also addressing other concerns. Because housing is essential for individuals to be able to work towards other goals, that is always your priority.
You also assist clients with addressing other problems such as benefits access, symptom management, educational goals, vocational goals, and more. Conscientiously, you monitor and evaluate each client's progress towards specific goals around housing, activities of daily living, and harm reduction. Additionally, you help to provide skills for behavioral health, medication management, and financial management. Making a difference one life at a time fills you with a deep sense of satisfaction, which is why you're perfect in this social services role!
QUALIFICATIONS FOR A CASE MANAGER This case management position will meet one or more of the following requirements:
Bachelor's degree in mental health, social services, or a related field, plus one year of experience relating to or assisting individuals with mental illness Certification as a qualified mental health associate through the Mental Health and Addiction Certification Board of Oregon or the ability to meet the criteria to be eligible for certification upon hire Lived experience plus a combination of two years of employment, education, or volunteer work Additionally, candidates are required to fulfill requirements set by the Laurel Hill Center, including:
Valid Oregon driver's license Willingness to undergo a background check and drug screening upon hire Full vaccination against COVID-19 Do you enjoy meeting new people and helping others?
