Responsibilities
• Conduct outreach to identify and engage clients in need of services• Perform psychological and needs assessments• Collaborate with clients to create and update personalized treatment plans• Facilitate connections with service providers and coordinate access to resources• Monitor clients' progress and adjust treatment plans• Advocate for clients to ensure their needs are met within the community• Provide counseling and crisis intervention• Offer daily living support to assist clients in achieving their goals• Supervise and monitor medication administration• Pass criminal history background investigation, DMV record check, Child Registry search, drug screening test, and employment reference checks
Requirements
• Bachelor's degree in Human Services• 1 year of experience in case management or service coordination services• Master's degree preferred• Registered with the Board of Counseling as a QMHP-A
