An RN in this position would be responsible for providing skilled nursing care to homecare patients, the team as a whole covers sections of Essex County, NJ such as;

Orange and Newark as well as some of the surrounding towns.

Responsibilities:
Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
Assesses and evaluates the needs of patients with multiple disease processes and co morbidities and develops an appropriate plan of care
Documents all changes in patients condition and updates care plans and outcomes
Identifies teaching needs and plans appropriate strategies for individuals and families
Monitors quality and appropriateness of services provided
Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
Adheres to agency infection control and safety policies
Adheres to the organizations policy in regards to absenteeism and appearance
Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications:
RN with Bachelors degree in Nursing preferred
1+ year of supervised clinical experience as a registered nurse
Excellent verbal and written communication skills
Strong computer skills
Excellent organizational and interpersonal skills
Car and valid drivers license
Working Conditions/Physical Demand

: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
