The Registered Nurse (RN) working as a member of an inter-disciplinary team is integral to Clinical Transplant Services. The RN functions as a liaison between the patients and staff members of the transplant team (physicians, social worker, financial counselor, psychiatrists, etc.) and has the primary responsibility for providing direct and indirect professional nursing care for patients. The RN provides Post-transplant care. The RN provides initial and on-going age and cognitive appropriate patient/family education in a variety of settings. The RN is responsible for the post-transplant call-in follow-up and transplant specific discharge education. After discharge, the RN is responsible for patient follow-up including clinic visits, diagnostic procedures, laboratory tests, biopsies, medication regimen plus age and cognitive appropriate education accomplished through frequent contact with the patients and families, the physicians, and other team members.
The RN assesses patient and family needs, develops a plan of care, implements and evaluates the care delivered, working closely with all members of the inter-disciplinary team. The RN is expected to practice in accordance with professional standards, core values, and with a commitment to service excellence.

Coordinate evaluation and waiting list needs with the inter-disciplinary team. Provide initial and ongoing transplant specific age and cognitive appropriate patient/family education, including inpatient education. Collect and evaluate pediatric specific existing lab, diagnostic, and consult results to determine if any additional testing as needed to determine transplant candidacy. Facilitate Committee Review presentations. Coordinate, collect and evaluate update testing. Waitlist management. Monitor on-going pediatric patient results, presenting abnormal results to the physician for review; execute physicians' orders, seeking clarification as needed. Coordinate admissions and outpatient infusions as needed. Coordinate biopsies Assess, educate and facilitate the patient's medication regimen; provide refills per protocol. Facilitate referrals to appropriate health care professionals. Triage and respond to patient phone calls assessing for potential or actual patient health problems, recognizing and responding to emergent situations. Document according to established policies and procedures. Prepare for and participate in post-transplant clinics both on and off site. Follow-up recommendations from clinic. Prepare for and participate in transition clinics. Develop, coordinate, and implement changes in the nursing process as it relates to transplant. Assume responsibility for professional/job related development. Incorporate quality improvement practices into performance. BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date. ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date. Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire Internal ONA applicants: Associate Degree in Nursing; BSN preferred Current, unencumbered Oregon State Registered Nurse License BLS at time of hire and must be issued by the American Heart Association (AHA) BLS must not expire during orientation Must be able to perform the essential functions of the position with or without accommodation
