This is a 12-hour shift three days each week, with the shift schedule being from 18:30 - 07:00, total of 36 hours per week. Monday to friday (3 days/12 Hours) Per Week: 36 Hours Roles & Responsibilities Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Note and carry out physician and nursing orders. Assess and coordinate patient's discharge planning needs with members of the healthcare team.
