We are seeking a Registered Nurse ( RN) Navigator to:
Bring your experience in pediatrics, pediatric rehabilitation and/or pediatric developmental disabilities. Coordinate admissions and assist with coordination of discharges. Coordinate family teaching about new rehab conditions, ensure smooth process for family from pre-admission through return to home. Collaborate with medical team and/or observe therapy sessions as needed to help determine patient/family education needs. Administer and review discharge surveys, lead QI initiatives, and assist with requirements for CARF accreditation. At UW Health, you will have :

Minimum - Bachelor of Science - Nursing degree (BSN).
Preferred - Masters degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field. Work Experience :
Minimum - Three (3) years of relevant clinical nursing experience.
Preferred - One (1) year of relevant experience in the specific patient population being supported. Licenses and Certifications :
Minimum - Registered Nurse license in the State of Wisconsin. Basic Life Support/CPR.
