Diabetic Educator- RN UnityPoint Health Fort Dodge FTE: .8 (32 hours per week, full-time benefits) Responsible for education to outpatients and other staff/community groups as requested by manager related to diabetes and existing population. · Assesses physiological, educational, psychological, sociocultural and spiritual needs of the infant, child, adolescent, adult and geriatric patient.
· Educates patients/families about diabetes and self management skills in collaboration with other team members, consistent with the mission, vision and core values of Trinity Regional Medical Center and the Standards of Care of the American Diabetes
Association (ADA).
· Assists to provide continuing education for the professional staff throughout the hospital.
· Participates in the annual planning/review of the program.
· Participates in performance improvement activities.
· Consistently demonstrates clinical competence in nursing procedures and patient care based on established theory and standards, maintains adherence to Unit Specific Standards and Centers for Medicare & Medicaid Services (CMS).
· Is a resource to patients/families calling office.
· Is a resource to physicians, staff, and the community and provides i nservices/educational programs as requested.
· Assists with planning, implementing and evaluating staffing patterns in accordance with level of education required ensuring continuity of care.
· Possesses an ongoing commitment to productivity by identification of new ideas to enhance efficiency.
· Builds and establishes positive working relationships with hospital staff, community agencies, the public and other members of the interdisciplinary team.
· Assists to coordinate/facilitate as appropriate interdisciplinary care planning.
· Assumes responsibility/accountability to respond to patient/family/physician needs.
· Assumes responsibility for personal and professional growth.
· Demonstrates positive customer relation skills.
· Travels to outreach locations to provide diabetes services and Thrive program in elementary schools.
· Assists with evening and weekend support groups/classes.
· Assists with employee health coaching program.
· Adheres to all department and hospital policies and procedures
Education: Graduate of an Accredited School of Nursing, BSN preferred. Refer to Voluntary BSN agreement.
Experience: Satisfactory performance as a staff nurse for a minimum of two years.
License(s)/Certification(s): Current RN Licensure in State of Iowa or applicable compact state. Must be licensed in state of residence. Valid Iowa driver's license. CPR required,refer to Job Description Addendum Certified Diabetes Care and Education Specialist certification preferred.
Knowledge/Skills/Abilities: Must demonstrate Demonstrated clinical expertise in diabetes education, leadership ability, knowledge, and effective application of teaching-learning methods as related to patient education. Ability to exercise good judgement, maintain effective interpersonal relationships, adapt to change and think creatively. Demonstrates knowledge and skills necessary to provide nursing care appropriate to infant, child, adolescent, adult and geriatric patients. Demonstrates effective communication skills and teamwork
