Responsibilities
• Coordinate patient care with the interdisciplinary team• Administer skilled nursing care in the patient's place of residence• Case manage and provide clinical direction• Implement, develop, and document patient plan of care• Provide care utilizing infection control measures• Assure continuity of quality patient care with appropriate documentation• Monitor assigned cases for compliance with third-party payer requirements• Demonstrate commitment, professional growth, and competency• Promote agency philosophy and administrative policies• Supervise and provide direction to home health aide and LPN
Requirements
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing• Current State license as a Registered Nurse, current Driver's License• Two years of experience as a Registered Nurse in a clinical care setting, home health or hospice preferred• Nursing skills as defined as generally accepted standards of practice• Good interpersonal skills• Proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption• Reliable transportation• Valid and current auto liability insurance
