$5,000 Sign-On Bonus after 90 days Our client, an Award-Winning Client that operates nationally and has garnered numerous accolades in the healthcare industry, is actively seeking a full-time RN Case Manager for Home Health. The successful candidate will demonstrate a high degree of nursing proficiency by providing excellent care and maintaining effective communication. The role requires an adept individual capable of operating autonomously with strong organizational acumen in various northeastern Ohio communities.
Required Qualifications: Associate degree or higher in Nursing At least 1 year of experience in any of the following areas: Acute Care Emergency Department (ED) Critical Care Burn Unit/Wound Clinic Oncology Medical-Surgical (Med-Surg) Home Health Care Current Registered Nurse (RN) license in Ohio (OH) Valid Basic Life Support certification from the American Heart Association (AHA BLS) Preferred Qualifications: Proficiency in Spanish Possession of additional specialized nursing certifications Key Responsibilities: Implement therapeutic, preventative, and rehabilitative nursing care Utilize the nursing process to assist in data collection, planning, and the evaluation of nursing care Communicate effectively with healthcare providers, patients, and families to ensure a collaborative environment Maintain patient safety and comfort Respond to emergencies based upon established nursing protocols Document patient observations and responses accurately and swiftly Provide skilled nursing care directly or indirectly to a diverse patient demographic Tools and Equipment Provided: Tablet Phone Schedule and Shift Details: Monday to Friday, 8:30 AM - 4:30 PM, including one weekend per month Work obligation includes one summer and one winter holiday Benefits: Comprehensive medical, dental, and vision insurance 401(k) retirement plan Health Savings Account (HSA)/Flexible Spending Account (FSA) Generous Paid Time Off (PTO) Additional Information: Two-week orientation session offered monthly Daily travel involving 5-7 patient visits Support available for appointment scheduling Note: Resumes will be maintained in strict confidentiality until such time as we engage in a detailed discussion regarding a specific opportunity, and both parties mutually agree to proceed.
