Title: Resource Navigator                                            Division: Project 180

Pay Range: $48,000 - $59,000/annually.
Revised: Summary
Under the supervision of the Program Supervisor or Program Manager, is responsible for but not limited to planning, facilitating, implementing, and coordinating service linkage to adults in the Rapid Diversion Program (RDP). RDP is a County-operated program that diverts criminal defendants out of the justice system and into services by providing in-court mental health evaluations, treatment linkage, and ongoing case management.
Essential Functions Screen and engage potential candidates for the Rapid Diversion Program Provide pre-diversion case management for clients based on their specific needs Research and establish connections with community resources and service providers to expand the pool of referrals available to the diversion team Connect clients with mental health, substance abuse, and other treatment programs as required Facilitate the process of accessing services, ensuring smooth communication between service providers and clients Maintain ongoing coordination with various public and private community health, mental health, and social service agencies to ensure clients receive appropriate services Build relationships and partnerships with courts, community organizations, service providers, and other institutions to expand the available resources for clients Engage in collaborative efforts, including interdisciplinary team meetings, to develop and implement individualized treatment and aftercare plans for clients Maintain and update client files and documentation in accordance with agency requirements Keep track of court dates and ensure that client linkages are completed promptly and in compliance with agency policies Regular attendance required Overtime, holiday, or weekend work may be required Perform other tasks and responsibilities as assigned by the Program Supervisor and/or Director    Secondary Functions Assist with other program activities as assigned by direct supervisor or program manager Minimum Qualifications Excellent communication skills Excellent interpersonal skills Ability to thrive in fast-paced environments Verification of employment eligibility and background check Maintain and uphold the agency's mission statement, values, policies, procedures, and principles Non-Essential Qualifications - Knowledge, Skills and Abilities
Bachelor's degree in a mental health-related field from an accredited college or university preferred One year of direct service experience in a mental/behavioral health setting which has an interdisciplinary team approach, preferred Compensation The salary range for this position is $48,000 - $59,000, depending on experience
Supervisory Responsibilities None Environmental Conditions Job duties are to be performed both in office and field settings
Physical Requirements  The employee is expected to sit, stand, walk, carry a maximum of 20lbs, listen, and speak in order to complete job requirements
Mental Requirements The employee must be able to handle the following: constant distractions, interruptions, uncontrollable changes in priorities and work schedules; be able to process information, think, and conceptualize
