About Agape of Appleton, Inc:
Agape is a Non-Profit (501c3) organization that provides residential support to individuals with differing abilities. Our mission is to continually enrich the daily lives of our participants by ensuring dignified and person-centered support in a variety of residential settings. As a Residential Case Manager at Agape:
You are responsible for case management, program management, supervising staff, and providing resident support as needed. The RCM oversees the day-to-day decision-making and has authority over their program operations. RCMs work under the direct supervision of the Services Coordinator (SC). Responsibilities: Provide (inter-intra agency) case management and support in a professional manner Develop and/or implement individual support plans for residents to assure individuals are receiving appropriate authorized care and services per contractual agreements Provide training, supervision, and guidance to program's Assistant Program Manager (APM) and caregivers Supervise staff interactions with individuals including but not limited to coaching, counseling, and mentoring Complete staff performance reviews and discipline as needed Work to maximize the social functioning of each individual and promote community integration/socializing Provide support and advocacy in an effort to safeguard their human and civil rights while fostering the dignity and worth of each individual Be knowledgeable of all State and Federal regulations pertaining to your program Provide support to individuals based on program needs (CBRF group homes tend to do more caregiving activities such as bathing, grooming, hygiene, dressing, mobility, and toileting. Our SAP (Supported Apartment Program) residents are typically more independent, and you will assist with shopping/errands, making meals, daily living cues, fun community outings, etc. You'll provide transportation, companionship, and conversation to residents.) Qualifications: Associate degree from an accredited college in a health-care related field or Bachelor's Degree from an accredited college and experience working in a health-care related field At least 2 years previous management experience is preferred Experience working with individuals in a group home setting is a plus Must be at least 21 years of age Maintain a valid driver license with excellent driving record Be mentally and physically capable of assisting participants based on their needs. Be able to use sound judgment in carrying out written and oral direction Ability to generalize and apply information in a variety of circumstances to maintain consistent support to individuals, especially during an emergency. Ability to be flexible and adaptable in hours of work, environments, and situations.
