Responsibilities
• Provide home-based care to patients after discharge from a healthcare facility• Work with a team of physicians and oversee LPNs, CNAs, and aides• Conduct patient exams, assessments, and document progress• Educate patients on care plans, medication, therapy, diet, and exercise• Coach family members on the patient's care plan and home safety• Ensure goals are met and coordinate discharge from services
Requirements
• Graduate of an accredited nursing program and active RN license in WI• Minimum of 1 year of nursing experience, preferably in home healthcare• Clinical proficiency in nursing specialty, strong communication skills, and compassion• CPR and ACLS certification
