Responsibilities
• Manage a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community• Work closely with patients and their families to develop and implement individualized care plans• Administer medications, perform treatments, and provide education and support to patients and their families• Collaborate with other healthcare professionals to coordinate and ensure continuity of care• Document and maintain accurate medical records• Adhere to all applicable laws, regulations, and company policies
Requirements
• Hold a valid Nursing License (RN) in the State of New Hampshire• Have a minimum of two years of acute care experience• Current CPR BLS Certification required• Reliable transportation• Home care experience preferred
