Behavioral Health Connections (BHC) in Phoenix is seeking a dynamic and talented Referral Management Clinician that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.
Job Duties/Responsibilities: Perform all aspects of the hospital's assessment and referral process. Service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, and systems complexities. and insurance coverage.
