PrimeCARE Level-One Referral Coordinator Job Description Referral Coordinators are expected to work as an integral part of the PrimeCARE team, helping to provide quality care to our patient in a time-appropriate manner. The following are the responsibilities, requirements, and expectations of a PrimeCARE Referral Coordinator:
Responsibilities Primary Level One Job-Specific Duties Review outgoing pending referrals to ensure correlating records are received from specialist appointmentsRequest records via phone or fax from outside facilities to complete the circle of care in outgoing referrals Access the fax inbox to retrieve referral requests and records for referrals Handle an appropriate quantity of work while doing a quality job Practice excellent telephone etiquette Use resourcefulness in problem-solving, staying organized amongst competing prioritiesMaintain compliance with HIPAA/OSHA rules and regulations; completing annual training in a timely mannerAssisting Level Two Job-Specific Duties Initiate, track, and appropriately document referrals/appointment requests, following up on pending referrals Assemble information concerning the patients clinical background and provide that information to appropriate requesting specialists and/ or facilitiesEnsure that referrals are addressed in a timely manner and that all effort has been made to ensure patient notification of scheduled appointmentsReview Telephone Encounters and contact patients when necessaryReview details and expectations about the referral to the patientSet others up for Success Anticipate, listen and respond to customer needs in a courteous, tactful, mannerFollow up and provide feedback in a timely and respectful mannerDemonstrate conservation and responsible use of resources; treat property with respectMaintain composure, great attitude, and moral integrity under pressureCommunicate effectively and work cooperatively with team members, vendors, and patientsPresent a positive image of PCMC through professional appearance and behavior Requirements High school diploma or equivalentExcellent customer service skillsOutstanding communication skillsAbility to read, write, and speak English according to proper grammar rulesPatience, sound judgment, and discretion in dealing with confidential informationOrganization and multitasking skillsExperience with computer data entryGeneral knowledge of MS OfficeWorking hours/schedule requirements Clinic Hours: Generally, Monday – Friday 8 AM – 5 PM, Unless otherwise specifiedFT: Work a minimum of 30 hours per week, up to 40 or above, depending on clinic needs.Preferred: Knowledge of EHR systemsExperience as a medical receptionist or similar positionKnowledge of medical terminology, regulations, and medical office proceduresWorking Conditions, Physical Requirements while performing the responsibilities of the job, these work environment characteristics are representative of the environment the job holder will encounter: Low-traffic work area, high level of patient and employee interactionQuiet to moderate noise levels from electronic/machine, telephone, and vocal noiseLimited to moderate risk of occupational exposure to blood-borne pathogens and other potentially infectious materialsVery occasional exposure to moving mechanical parts (i.e., fax machine toner cartridge replacement, etc.)Employee will be required to speak to and listen to othersEmployees will be required to process, remember, and comply with changes in processes and protocols with or without warningMajority of time may be spent sitting in front of a computer, using hands and fingers to perform job tasks; some reaching is required; many job functions require repetitive motionVision abilities required to perform this job include close vision and peripheral visionEmployees will rarely be asked to lift anything above 25lbsAttendance will be required at staff meetings on a monthly basis outside of regular working hours (generally, early morning)Travel may occasionally be required to attend meetings or to cover for teammates at other locations (generally, at other PrimeCARE clinic locations)Employees are strongly encouraged to attend non-mandatory company events for marketing and team-building.Reasonable accommodations may be made to enable people with disabilities to perform the job essential functions.
This job has no supervisory responsibilities. Employees in this position will report to the Corporate Support Manager. Supervision is
minimal to moderate and employees are expected to help each other comply with policies and procedures.
