Title:RBE Network Coordinator II
Location: Downtown Chicago (30 W. Monroe St.)
Role Description: Oak Street Health will be launching a new team of network coordinators to support communication between a newly formed Risk Bearing Entity (RBE) and other providers. The Coordinator, Network Coordination (RBE) is a newly created role that will support across multiple areas including: reaching out to our PCP network to validate information and fill gaps, ensuring medical records are shared in a timely manner between the RBE and PCPs, and supporting specialist referral processing needs.
The role will report to the Supervisor, Network Coordination (RBE)
Core Responsibilities: Conducts outbound verification calls to speciality provider offices and facilities
Validate and update address, fax number, insurance acceptance, and other agreed upon data to better power Oak Street Health's database and tools
Document the information collected in a detail oriented manner, with no spelling errors, typos, or missing information
Responsible for processing referrals and diagnostic orders and obtaining required authorization where necessary
Package relevant patient records from RBE support services and share with PCP of record
Request necessary patient medical records from facilities and provider offices
Encourage the teammates around you to deliver excellence and improve the end results
Provide the highest level of customer service when interacting with provider offices and facilities
Other duties as assigned

Experience in health plan and/or clinical reception is preferred, but not required
Organized and able to manage competing priorities
Detail oriented and an ability to document in a high-quality manner

Resourcefulness in problem solving
Proficient PC skills, including basic Microsoft Excel / Google Spreadsheets skills
U.S. work authorization
