The Rapid Rehousing Specialist (RRS) will assist homeless individuals and families transitioning from the emergency shelter into permanent housing. The Rapid Rehousing Specialist reports directly to the Shelter Manager.
Essential Duties Work closely with the Shelter Manager, Case Managers to identify housing resources for clients at shelter. This will include families, single men, and single woman. Develop and implement appropriate comprehensive individual service plans to assist clients in obtaining housing within 90 days from entering shelters; this includes conducting assessment interviews; providing assistance in identifying housing, assisting clients in completing all necessary application forms and providing this information to participants; assist in securing necessary documents needed to acquire permanent housing; working with case managers in obtaining and maintaining self-sustaining sources of income, benefits, and other economic supports as well as professional resources to assist the client in reaching jointly established goals. Work closely with Shelter Managers, the Resident Intake Coordinator and Case Managers in establishing housing goals and monitoring those goals on a monthly basis. Works in teams and triages client cases based upon the most urgent need and length of stay criteria and providing documentation of all team meetings and case management. Assist clients in identifying community and personal resources; support clients in making necessary appointments with community resources and arrange transportation required for meeting appointments. Cultivate relationships with local landlords; develop program literature for landlords; become familiar with housing databases; conduct events to educate landlords; maintain list of units and landlords; follow up on placed residents with landlords and rent payments. Participate in Homeless Roundtable, Homeless Coalition meetings and other meetings as assigned by Homeless Services Director; actively collaborate with goals and objectives set forth in these Committees and other local agencies that serve the homeless and provide housing. Arrange and coordinate for workshops that might help clients with specific life skills; collaborating with community resources, churches, and volunteers to assist clients in obtaining services that would increase sustainability of housing (e.g., budgeting workshops, banking, homemaking skills, etc.). Set up home visits and/or telephone contact with clients. Assess and evaluate the housing services being provided in order to ensure service outcomes are favorable and meet or surpass funding requirements.
