Philadelphia FIGHT Quality Improvement Associate Job Description A crucial element to providing state of the art health care is quality improvement. The Quality Improvement (QI) Associate is responsible for planning, implementing, and evaluating the quality, patient safety, and operational performance program of our community health centers. The QI Associate serves as a change-agent for entity-wide practices and supports teams connected to those areas. The QI Associate provides partners with the most current performance improvement (PI) methods and tools, including strong project management support. This position is accountable for facilitating improvement in the areas of patient satisfaction, clinical quality, and patient safety, operations, and continuous survey readiness.
Qualifications: The QI Associate will have a bachelor's degree from an accredited four-year college or university; or three to five years related experience and/or training; or equivalent combination of education and experience. Six Sigma Black Belt/Lean Certification/IHI Improvement Advisor training [e.g., Six Sigma PDSA (Plan Do Study Act) or PDCA (Plan Do Check Act)] or equivalent experience (e.g., leading or facilitating large scale projects across multiple areas and familiarity in process improvement methodology).
Responsibilities include: Mentoring, teaching, and coaching teams, providing feedback, support, and direction as needed. Provide health centers and partners with the most current PI methods, assisting in seeking areas for improvement and the facilitation of projects. Able to competently evaluate the quality, patient safety, and overall performance of selected health centers. Facilitates improvement activities to insure safe, effective, and compliant patient services. Serves on the core team of the Continuous Quality Improvement Committee and represents a structural quality improvement perspective. Schedule and chair all quality management meetings including developing an agenda, taking thorough meeting minutes, and helping the health center teams develop and follow through on Quality Improvement Projects including the development of clear action steps. The Advisor will also support health center staff in between meetings to work towards QI goals. Collaborates with leadership & frontlines to effectively prioritize problem solving. Collaborate with leadership to understand program details and revenue generation to strengthen the organizations position for quality incentives and shared savings. Collaborate with health center leadership to support the design and updating of health center workflows. Identifies gaps in efficient workflow adoption at all practice sites of Philadelphia FIGHT Community Health Centers. Maintain the "Strategic and Quality Management Plan" and ensure its monitoring and completion of tasks listed within. Participate in risk management in coordination with the CCO, CQO, and the Director of Risk Management. Serves on morbidity and mortality conference task force. Able to create and generate reports, in collaboration with informatics team, to insurances, city, state, and federal regulatory bodies including all the performance measures and data reporting requirements including but not limited to Meaningful Use (MU), UDS, PCMH, HEDIS, and Ryan White HIV/AIDS program Services Report (RSR). Helps teams and leadership access useful data. Helps the organization understand trends, the meaning of data, and how it compares to benchmarks. Brings tools to teams and leadership that are useful for data analysis and dissemination. Is effective in disseminating information. Maintains data integrity & validity. Develops reports for monthly presentation to the Board of Directors by CQO. Proactively seeks out opportunities to improve the patient's experience. Work with teams to create customer loyalty and satisfaction. Coordinate Patient Satisfaction Surveys in accordance with regulations to improve patient experience. Uses Patient Satisfaction Survey to work with health center leadership to drive change based on survey results. Responsible for incorporating patient feedback, through various means, into quality improvement and risk management processes. Other: Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
Salary range: Competitive; Hourly
Travel Requirements: Minimal local travel required between Philadelphia FIGHT Health Centers.
Work Hours: 40 Hours/Week (M-F 9:00am - 5:00pm)
