Responsibilities
• Coordinate care to ensure continuity of care for clients• Provide screenings and case management• Participate actively as a team member in the delivery of client services• Assist in ensuring agency compliance with laws, regulations, and contracts• Ensure medication storage, maintenance, dispensation, and waste logs are maintained• Follow medical, nursing, health & safety, and personnel policies and procedures
Requirements
• Must possess a Certified Nursing Assistant (CNA) License from Washington State• Must have a current CPR/BLS card or obtain one within 90 days of employment• Must have a current First Aid Certificate• Basic mathematical skills are required• Must have no history or evidence of alcohol or other drug misuse for the past 3 years• Washington State Driver's License required• Familiarity with computer software desired• Bilingual in English/Spanish preferred but not required• Must be able to pass a background screening
