Transformation projects and programs - in collaboration with Department of Family Medicine leadership and other primary care departments. Responsible for project management and implementation of the initiative's goals and objectives for all transformation projects and programs. This includes logistical, analytical and technical support, meeting and reporting on milestones. Provides assistance to clinics to ensure requirements of receipt of supplemental program payments. Manage clinical Quality Improvement projects in coordination with Clinical Leads in Quality Improvement and Informatics. Manage work tasks and priorities of Care Coordinators as a support team to clinical workflow, patient services, and outreach. Guide and oversee team-based projects & pilots. Supervise staff who provide administrative services to the clinic to prioritize tasks, review scope of work & new requests, review work-related performance, and provide problem resolution. Provide new provider and/or resident orientation and panel reallocation. Staff includes Administrative Coordinator, Administrative Assistant, and Medicaid Specialists Areas of responsibility includes management of the following: Population Health management, report requests & development, financial management, risk stratifications, assist with the Patient Advisory Council, Leaming Collaborative with Community Partners, Quality Improvement initiatives, Research projects, Grant funding requests and meeting milestones.
General FM Operational Standardization: Development and implementation of standard policy and procedures as agreed upon by managers and medical directors (travel clinic, call routing, MyChart work flows Monitors and manages access per clinic and panel size per providers. Works proactive with management and staff provider template management. Assist in patient satisfaction improvement Ensure the FM website has the latest policy and procedures are posted up and up-to-date. Train staff to utilize this resource Short-Term Projects - Support for Vaccination Clinics, operations, and Volunteer Coordination. Financial: Ensure completion, submission and tracking of payments for the fiscal year budget. Maintain open dialogue with payers to ensure coordination of efforts and clear communications. Review patient lists for each clinic and perform individual chart review to ensure accuracy and reimbursement for eligible patients; Reports submitted quarterly. Facilitate grant and other funding requests for the clinic. Population Health & Risk Stratification: Assist in the creation and maintenance of risk stratification process Train, monitor, and maintain workflow to assign risk stratification to every empaneled patient in EMR Ensure risk stratified patients are appropriately indicated in the EMR Track metrics on percent assignment of risk status to proportion of the population for each risk category Report which risk stratified patients ha~e received care management services Provide reports & guidance on priorities and metrics for Population Health management Quality Improvement of Clinical Care: Ensure deadlines are met by providing necessary examples and date Participate in development of workflows as appropriate Attend Family Medicine Data Team Meetings Ensure care plans and action plans fulfill specific program requirements - Develop and manage an "ideal" care transition model with a method for monitoring its achievement by percent Collaborate with QI & Informatics Clinical Leads to optimize and prioritize clinical needs. Assist in project coordination of Resident QI projects. Four years of experience as an executive assistant, administrative analyst, researcher or administrative officer which included administrative duties, provision of technical assistance, or operations review. Two of the required years must have included administration or oversight of an on-going project or program;  A Bachelor's degree in Business Administration, Management, Public Administration or a closely related field may be substituted for up to two years of the required experience.  Note : there is no substitution for the two years of administrative experience. Job Related Knowledge, Skills and Abilities: Excellent communication skills Computer skills, including EMR Demonstrated ability to work with a variety of diverse customers Skill in developing and maintaining professional relationships Work independently and collaboratively Ability to prioritize work demands Ability to interpret and follow policies, procedures and protocols Basic applied math skills, including calculations and conversions Microsoft Office including Excel and Office apps Must be able to perform the essential functions of the position with or without accommodation. One year of work experience in a medical office setting including high volume direct patient contact and use of EMR with understanding of healthcare transformation. Understanding of current Healthcare Reform requirements and expectations. Family Medicine at Scappoose; some travel involved for meetings as mentioned above.
