Job Description. RLS is hiring a Program Coordinator to work at our Adult Foster Care homes in Grand Rapids. We provide a residential setting to individuals with Mental Health diagnoses who need 24-hour supervision. We assist our residents with daily living needs, help them develop independent living skills, and provide support. The Program Coordinator (sometimes called the House Manager or Manager) position is a full-time position where the minimum expectation of work would be 32 hours a week. Providing direct care, support, and oversight of the day-to-day operations of their respective persons residing in that Residential Support Service home as well as the employees assigned to that home.
Essential Qualifications: All applicants must maintain qualified status under Minnesota 245D (Criminal Background Studies of individuals affiliated with Programs Licensed by Department of Human Services, a valid driver's license with a clean record, and required annual training). At the end of the orientation, all employable employees must successfully pass written and practical testing of training material, become certified to administer medications to RLS individuals, and be First Aid certified to be considered for RLS AT-Will employment. This is a leadership role and this person has shown exceptional communication and computer skills and a strong knowledge of the Direct Support Professional staff position. Ability to respond quickly to phone calls, texts, or emails regarding requests and expectations from supervisors. Reliable and personal transportation that is insured in the state of Minnesota. A driver's license is required. The successful candidate must be cleared by a DHS Background Study. Education/Experience Required: A minimum high school diploma. One year of experience working directly with individuals with disabilities. Successful completion of RLS Orientation. The knowledge, skills, and ability to successfully complete the duties of the job. Read, write, and speak the English language in an understandable manner. Knowledge of Word, Excel, and Outlook systems. Ability to utilize a fax machine, phone system, document scanner, and other office materials as necessary for the position. Responsibilities and Job Duties may require, but are not limited to: Provide quality support designed to enhance each person's skill development, health, freedom, and personal dignity. Promote community participation, person-centeredness, and an approach that focuses on supporting persons receiving services in the most integrated setting. Oversee the activities of DSPs to ensure individuals receiving services are in the community on a regular basis. Maintain an appropriate therapeutic relationship with each family, guardian, and individual to ensure the quality of services provided by RLS. Communicate effectively with guardians to ensure services and supports are discussed, reviewed, and approved. If assigned in the CSSP or CSSP Addendum, ensure medical appointments are scheduled and followed up with qualified staff. Oversight of the license holder's responsibilities assigned in each person's Coordinated Service and Support Plan and Coordinated Service and Support Plan Addendum. Ensure that monitoring and PRN mediation recommendations following incidents and/or medical procedures are followed accurately per the medical professional's order. Ensure MARs, medication supervision, and self-administration of medication are completed and documented accurately. Ensure any medication changes or updates are reflected immediately on the person's MAR. Communicate all Doctors' orders and document all medical communications in the Health Progress Notes. Ensure the completion of all documentation during each shift on a daily basis, such as the completion of daily progress notes, health program notes, observations, MARs, and communications logs. Ensure that DSPs receive ongoing training by conducting DSP team meetings and by offering emergency procedures, behavioral, medical, and program implementation/revisions. Take responsibility for the performance and skill growth of DSPs by being available, giving regular performance feedback, and providing additional training on incorrect work practices, both individually and in group settings. Supervisors may share and rotate call responsibilities. This rotation typically involves alternating weekends with other house PC, DC, and DM. These weekends are scheduled ahead of time and submitted by the deadline to the main office. The rotation is never a guarantee, as exceptional circumstances may occur. Comply with weekend on-call rotation and conduct visits within the hours randomly during an on-call weekend. Will be required to work in an unscheduled emergency situation if all DSP staff are unavailable. This position is a full-time position of 36-40 hours and requires you to be available via phone or text much of the time and requires you to be flexible in your schedule. You are required to respond to those calls and handle any urgent matters, emergencies, etc. that arise. You may need to work evening and weekend hours in an emergency and must be able to do so on short notice. Other tasks & duties as assigned. Job Location: Residential Living Solutions Inc.
