Announcement # Jefferson Healthcare () is looking for a Population Health Specialist to join our team. Our Population Health Specialist will be supporting the design and implementation of population health programs and projects and will monitor, manage, attest, and ensure successful reporting to the assigned programs. Core duties will consist of program development, external communication as well as more general responsibilities. Reporting to the Chief Planning and Advocacy Officer, the Population Health Specialist will collaborate with and engage senior leadership, administration, professional, and other staff throughout the organization to implement the population health program and promote a culture consistent with the Jefferson Healthcare's mission and values.
Some job duties include:
Designs and evaluates population health interventions using evidence based and promising practice. Collaborates with the Quality and Patient Safety and the Care Transformation departments in the development of the population health program. Under guidance of the executive director, develop key community relationships for program design and implementation. Maintains project documentation Represents Jefferson Healthcare population health interests with external parties. Develop and implement community outreach initiatives to enhance community engagement Identify key stakeholders, such as community leaders, organizations, and influencers, and build relationships with them to foster positive partnerships. Collaborate with marketing and communications teams to create and execute community-related content and campaigns. Work closely with providers to achieve key measures of success Learns, understands, and assists with workflows within the EHR as necessary to meet program objectives.
