Outpatient Orthopedic Skilled Nursing Facility Acute Care Schedule: Full-time, 80 hours per pay period
Position Summary: To carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize the patient's progress toward achieving functional goals. To maintain a positive working relationship with other health professionals and ancillary staff.
Position Responsibilities: Provides direct patient care, evaluates functional needs and outcomes, consults with other specialists as needed Reviews patient's functional needs and adjusts care plan as indicated to ensure optimal patient outcome. Designs and implements a plan of care for the patient based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate. Conducts therapy practice within defined standards of care. Directs the assistance of support staff in order to achieve the patient's functional goal. Communicates effectively with other professional and support staff in order to achieve positive patient outcomes. Practices established hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards. Documents evaluations, treatment goals, and plans using appropriate electronic medical software, in a timely fashion. Regularly updates documentation and maintains all required prescriptions and signatures. Enhances professional growth and development of self and others through participation in education programs, in-service meetings, and workshops. Keeps up to date on current professional literature. Provides direction and leadership to staff and acts as a resource to promote physical therapy within the community. Position Requirements: Minimum Education:
