Pharmacist - Part-Time

Kalamazoo Long Term Care Pharmacy is looking for an experienced Pharmacist looking for their next challenge to work on-site at PACE Central Michigan in Mt. Pleasant, Michigan and Sunrise PACE in Fort Gratiot, Michigan.
The PACE Pharmacist Liaison works closely and effectively with Technician, Primary Care Providers, Nurses, other PACE staff, and participants to ensure the Clinic's smooth operation regarding medications. The Technician represents KLTC on a daily basis and builds trust and strong relationships between the Pharmacy and PACE program.
Job Responsibilities: The Clinical Pharmacist assumes responsibility for the day to day comprehensive, customer oriented clinical pharmacy services provided by PACE as well as medication utilization and project management under the supervision of the Clinical Pharmacy Manager. The Clinical Pharmacist works in collaboration with the Medical Director and reports to the Clinical Pharmacy Manager. The Clinical Pharmacist must be able to exercise sound management and judgment in making decisions for optimization of drug therapy in elderly participants. In addition, he/she collaborates in PBM oversight, ensuring adherence to Part D regulations and cost-effective medication utilization. He/she assists the PACE interdisciplinary team in identifying and resolving drug-related problems for participants. The Clinical Pharmacist works closely with the corresponding operational pharmacy facility to help ensure accuracy and avoid gaps in care. The Clinical Pharmacist participates in training other health care professionals in the service area. They work closely with medical and administrative leadership as well as primary care providers to develop and implement strategies which continuously improve the quality of participant care at the best value for participants and the organization.
