Credence Management Solutions, LLC (Credence)  is seeking a Patient Safety Assistant Coordinator (PSAC) to support the Air Force Patient Safety Program (PSP) and perform data analysis functions at the Travis, AFB Military Treatment Facility (MTF) in California. The PSAC responsibilities includes assisting, advising and making recommendations to the MTF leadership pertaining to administration, management and daily operation of the program. These recommendations includes adherence to established policies and procedures, as well as identification of risk reduction strategies from analysis of healthcare information. This postion is based on site at the 60 th Medical Group – Travis AFB, CA.
Full salary range for this position is $82,000 to $88,000 per year, with the starting salary determined based on candidate's knowledge, skills, experience, as well as budget availability.
Collaborate with the Patient Safety (PS), Risk Management (RM), Quality Management (QM), Process Improvement personnel and other related function teams regarding all aspects of the PSP, with emphasis on performing data analysis. Assist, advise, and making recommendations to the MTF leadership to develop, coordinate and execute Proactive Risk Assessment (PRA) plans to identify, reduce and prevent, wherever possible, risks to safe care. Patient Safety (PS) activities under this plan include but are not limited to: Preventable medical event and near miss reporting Analysis of near miss and lesser harm events Root Cause Analysis (RCA) on higher severity events and others as indicated Proactive Risk Assessments (PRA), communication and team training Reporting to the DoD PS Center. Assist the MTF team with identifying key performance measures directed at risk identification, reduction, and prevention. Communicating these performance measures to MTF leadership, appropriate committees, functions and individuals. Serving as a PS resource to for both programmatic and data analysis functions and conferring with all levels of personnel to assist with the development of program plans directed at risk identification, reduction and prevention to decrease both the frequency and severity of preventable medical events and harm to patients. Assist MTF staff with data collection and analysis of PS-related PI projects. Serve as a resource to ensure compliance with DoD mandated analysis and patient reporting systems such as TapRooT® and the Joint Patient Safety Reporting System (JPSRS). Provide consultation on how the MTF uses the collected data to identify system and process issues that could have or did result in harm. Assist the MTF team to clarify the classification of each reported event and make recommendations to determine the level of assessment required. Search existing healthcare information sources and research clinical findings for PS relevant issues. Assist with developing and executing mechanisms to translate this data to information and providing this information to leadership, committees, and individuals as appropriate. Educate and assist Military Treatment Facility team in identifying safety improvement opportunities from data/information sources. Assist with the facilitation of the RCA teams through completion of the Root Cause Analysis (RCA) process and debriefing the MTF Commander or designee when an RCA is designated by the MTF. Analyze complete RCA's for the MTF for opportunities to improve the process, lessons learned and causal and contributing factors. In collaboration with the PSM, make recommendations to the MTF for facilitation of future RCA teams. Assist MTF to develop and use tracking system for RCAs. Assist the MTF in PS rounds throughout the MTF. Communicating directly with MTF staff to identify and recommend opportunities to affect safer care. Encouraging PS event reporting throughout the organization. Participating in preparation for The Joint Commission (TJC) and Unit Effectiveness Inspection (UEI) as defined by the MTF. Collect, collate, analyze and display data from event reviews, Root Cause Analysis, PRAs and other sources, as designated by the Chief of Medical Staff (SGH) and disseminate to appropriate committees and individuals for PS improvement purposes. Assist in preparing and presenting briefings on MTF PSP trending and reporting MTF findings from PSR reports as appropriate. Assist in the development, coordination, and presentation of ongoing PS education in the form of new employee orientation, one-on-one consultations, in-service, and just-in-time training. Attend meetings and other designated functions to ensure coverage by the Quality Management personnel. Maintain pertinent knowledge through review of current PS literature and attendance at professional PS meetings and educational programs Bachelor's degree, preferably in a healthcare-related field, Business Administration (BA), Information Systems (IS), or equivalency in degree programs that include data analysis and statistical analysis. Two (2) years of experience at a healthcare facility. Experience in a civilian inpatient or outpatient medical facility. Must have strong skills utilizing Excel. Proficiency in Microsoft Suite of tools including Outlook, Word and PowerPoint is required. Understanding of Microsoft Access, Visual Basic, and SQL is preferred. Must possess ability to work independently, organize time, and manage multiple projects simultaneously Attention to detail and the ability to effectively communicate Patient Safety concepts to leadership. Knowledge of The Joint Commission accreditation standards, process, Patient Safety initiatives and patient safety goals is preferred. Experience with performing Root Cause Analyses, Proactive Risk Assessments  and Process Improvements is highly desirable but able to be acquired after start. Strong writing and presentation skills. Effective team and interpersonal skills. Must possess ability to work collaboratively with  Patient Safety (PS), Risk Management (RM) and Quality Management teams. Military healthcare experience desired.  Working Conditions and Physical Requirements:
