We are actively looking for compassionate team members who desire to help our clients remain independent and have their best life now!Job SummaryA registered professional who provides coordinated care to patients of all age groupsPlans, implements and evaluates patient care plans to restore or maintain patient well beingEvaluates occupational roles and occupational performance of patients with physical, psychosocial, and/or cognitive impairmentsProvides intervention to deficits and elicits responsesCollaborates with the interdisciplinary team.Essential FunctionsPerform initial and ongoing clinical assessments to identify patient needs, and determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home healthDevelop and revise a Plan of Care in consultation with the physician and other care team membersProvide treatments, evaluate the need for and use of adaptive equipment and environmental modifications, fabricate/maintain adaptive equipment and hand splints as applicableEvaluate outcomes of care with appropriate discharge planningApply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of diseaseAccept clinical assignments that are consistent with education and competence to care for patientsSupervise COTA and HHA as appropriate. Complete appropriate documentation in a timely mannerMeet mandatory continuing education requirements of the agency/licensing boardUse effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriateDemonstrate commitment, professional growth, and competencyPromote agency philosophy and administrative policies to ensure the quality of careQualificationsEducation: Bachelor of Science in Occupational TherapyLicensure: Current Texas State License as an Occupational Therapist, current Texas Drivers LicenseExperience: Two years experience as an Occupational Therapist in an acute or rehabilitation settingSkills: Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of CPR and Hepatitis profile.
