Nurse Practitioner Or Physician Assistant Needed For Internal Medicine

(Only QUALIFIED Healthcare Professionals accepted) Nurse Practitioner or Physician Assistant - Nurse Practitioner or Physician Assistant needed for Internal Medicine or Primary Care role with a Community Health Group near New Haven, Connecticut. APRN and PA providers with 1-2 years of relevant outpatient clinical experience will be considered. Medical Spanish a plus. This position is available as of 5|6|2024.
Nurse Practitioner | Physician Assistant - Roles | Responsibilities:
- Provide Primary Care | Internal Medicine services to Adult patients at 1 outpatient clinic setting near New Haven, Connecticut.
- Coordinate with other clinical and non-clinical staff to provide optimal patient outcomes.
- Manage acute and chronic conditions for Adult patients.
- Prescribe medication for patients as required or directed.
- Coordinate with specialists.
- Provide education to patients and family members when necessary.
Days | Hours | Compensation | Benefits:
- All preferred APRN and PA schedules will be considered
- Scheduling is based on 4hr shift blocks (ex: 24hrs|wk = 6 x 4hr shifts)
- All PA and APRN Providers may work their desired number of 4hr shift blocks.
