Position Overview: We are looking for a dynamic and autonomous Nurse Practitioner to take on the role of Program Manager for our Wound Care Program. The successful candidate will play a crucial role in overseeing and managing the program, providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.
Responsibilities: Clinical Leadership: Provide direct patient care through all available channels and products. Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices. Monitor and evaluate patient progress, adjusting treatment plans as needed. Program Management: Oversee the day-to-day operations of the Wound Care Program. Collaborate with interdisciplinary teams to ensure seamless patient care coordination. Develop and implement protocols, policies, and procedures to enhance program efficiency and effectiveness. Chronic Care Planning: Develop personalized chronic care plans for patients with wounds, focusing on long-term management and prevention of complications. Educate patients and families on self-management strategies and lifestyle modifications. Communication and Collaboration: Establish and maintain effective communication channels with referring providers to ensure continuity of care. Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care. Qualifications: Current and unrestricted Nurse Practitioner license in AZ. Certification as a Nurse Practitioner with a specialization in wound care management preferred. Minimum of 1 year of experience in wound care. Strong interpersonal and communication skills.
