The selected provider will be stationed at either the Emergency Room at Crownpoint Healthcare Facility, the Adult Ambulatory Care Clinic in Crownpoint, or at Thoreau Health Station or the Pueblo Pintado Health Clinic.
Crownpoint Healthcare Facility (CHCF) is a small rural hospital offering services such as a non-designated trauma-level Emergency Department, Emergency Department Fast Track, inpatient care, and various ambulatory care clinics. The CHCF Emergency Department operates 24/7.
Expected Schedule:
The provider will adhere to the hours specified by the Crownpoint medical staff schedule. Emergency Department shifts may span from 10 to 12 hours per day, starting between 8:30 am to 9 am. Clinic hours are negotiable, ranging from 7:30 am to 6 pm or 8 am to 4:30 pm, depending on facility requirements, including any last-minute changes to cover unexpected absences.
Contractor Qualification Requirements:
Maintain a valid, current, full, and unrestricted license to practice medicine in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico throughout the contract term.
Possess Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Advanced Cardiovascular Life Support (ACLS) certifications.
Obtain board certification, either from the American Association of Nurse Practitioners (AANP) or the Family Nurse Practitioner Certification (FNCP).
Provide proof of an active and unrestricted NP License and DEA license.
To meet the credentialing standards within the Indian Health Services, all providers must:
Hold full and unrestricted licenses in all states where licenses are held.
Have no history of adverse Medical Staff Action or license revocation.
Be free from criminal arrests or convictions.
Have no record of censure by a medical board for opioid use or misuse or sexual misconduct.
Have never been terminated early from a Locums Assignment due to behavior or quality of care.
Emergency Room Duties:
The Nurse Practitioner will handle acute/urgent issues such as toe nail removals, simple laceration repair, splinting of fractures, and incision and drainage. Additionally, due to limited availability of Adult Medicine primary care providers, our fast track ER handles a considerable amount of what would be considered primary care, including urgent referrals and medication refills. The NP must proficiently enter referrals into the Electronic Health Record (EHR) as per service unit policy.
The NP must adhere to service unit policy regarding chronic pain medication. Apart from diagnosing and treating medical conditions, the NP will complete all billing and encounter forms, including discharge instruction sheets. Charts must be completed within four (4) days of the patient encounter.
NP agrees to attend ER staff huddles at 10 am on normal workdays and may be assigned to participate in triage by performing brief medical screening exams in the triage room as determined by the ER charge nurse.
Clinic Duties:
The contractor will assist primary care teams by serving as a Same Day Appointment provider for urgent needs, medication refills, and referrals. This requires the ability to diagnose and manage common adult medical conditions such as hypertension and diabetes.

The Indian Health Service (IHS), an agency within the U.S. Department of Health and Human Services, is responsible for providing federal health services to American Indians and Alaska Natives. The IHS is the principal federal health care provider and advocate for Indigenous peoples, aiming to assure comprehensive, culturally acceptable personal and public health services are accessible to them. Currently, IHS provides health services to approximately 1.5 million American Indians and Alaska Natives across 34 states, addressing a nationwide shortage of medical providers in certain specialties, particularly in remote and rural areas.
Health Requirements:
Immunizations: Immunity to Rubella, Mumps, Measles, Hepatitis B series, Varicella, Tetanus Diphtheria (Td), Pertussis, and Influenza within specified periods.
Documentation of receiving a TB Mantoux skin test (PPD) within the past 12 months with documentation of follow-up for a positive test.
A signed declination of the Hepatitis B vaccination series will be accepted.
