Responsibilities
• Provision of exceptional primary care• Conduct routine and acute office visits• Administer annual wellness visits and health risk assessments• Care coordination with other providers• Population health leadership• Assist with phone triage and outreach• Educate patients on health conditions, care plans, and treatments• Participate in promotional activities• Conduct home visits as needed• Other assigned duties
Requirements
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate• National certification in at least one relevant specialty• Active state Nurse Practitioner license• Active DEA license• US work authorization• Experience in primary care• Understanding of HCC documentation, ICD-10 Coding, and HRAs• Passion for teamwork and collaboration• Desire to be part of an innovative model focused on population health• Bilingual proficiency in applicable areas
