Responsibilities
• Work as part of a clinical care team to deliver care in the patient's home• Perform comprehensive health assessments and diagnose complex illnesses• Provide therapeutic interventions and minor procedures• Document patient encounters and analyze test data• Educate patients and families on acute illness management• Navigate end-of-life conversations• Communicate effectively with patients, family, and care providers• Identify and solve problems• Adhere to clinical and safety standards• Maintain positive relationships and attend training sessions
Requirements
• Minimum of two years of experience as an APP in relevant areas• Graduate with an advanced degree from an accredited institution• Current unrestricted state license as a Nurse Practitioner or Physician Assistant• Nationally board certified• Current BLS required, ACLS certification preferred• Prescriptive authority and DEA• Valid driver's license with clean driving record• Complete required drivers training upon hire• Ability to lift and carry equipment up to 50 pounds• Ability to walk up and down several flights of stairs easily• Flexible to work evenings, weekends, and holidays
