Roseburg is located on Interstate 5 in Southern Oregon's scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. Responsibilities As a Hospice Admissions Nurse you will perform hospice adminssions and develop plans of care.
Essential Duties:
Asseses and develops a plan of care through interviews of patient, primay cargivers and family. Provides and documents skilled care, treatments and diagnostic procedures. Makes referrals and coordinates services. Shift: 9am - 5pm
Core Schedule: Monday - Friday, Includes On Call
Qualifications License/Certifications:
Current State of Oregon RN License American Heart Association BLS for Health Care Providers within 30 days Valid Oregon Driver's License and automobile insurance Experience:
2 years of acute care, Home Health or Hospice nursing experience Home Health and/or Hospice experience preferred
