PRN After Hours On Call Home Health Registerd Nurse
Work Schedule 5:00 PM to 8:00 AM Monday through Friday and or 24 hours on Saturday and Sunday
$45.00 to $50.00 per hour DOE, four hour minimum pay per day

This position is an RN who works outside of scheduled business hours. This person is responsible to be the field contact for Triage nursing and responds to calls as they come in while on shift. This position is responsible for completing patient visits and any required follow-up for patient/caregiver needs as well as any scheduled visits after business hours. They may plan, organize, and direct Hospice and/ or Home Health care as needed. The nurse builds from the resources of the community to plan and direct services to meet the holistic needs of patients and families with an emphasis on palliative care for hospice patients and rehabilitative care for home health patients.
Responsibilities:
Patient Care:
Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
Assesses and evaluates patients status by evaluating patient and family/caregiver needs, as directed and participating in revising the plan of care in collaboration with the attending physician (or approved medical provider), and/or the hospice medical director as the patient status and needs change.
Works autonomously to initiate appropriate preventive, rehabilitative or palliative care nursing procedures. Administers medications and treatments as prescribed by the physician (or approved medical provider) in the patients plan of care.
Involves the patient and family/caregiver to address needs and meet related
Provides health care instructions to the patient as appropriate per assessment and
Assists in facilitating the patients efforts toward self-sufficiency or obtaining appropriate caregiving and optimal comfort care or independent level of functioning.
Communication:
Documents patient services and status timely and accurately within professional practice standards and reimbursement requirements.
Records pain/symptom management changes/outcomes as
Communicates with the physician (or approved medical provider) regarding the patients needs and reports changes in the patients condition; obtains/receives physicians( or approved medical provider) orders as required.
Collaborates with the on-call hospice provider to change the patients level of care as appropriate and indicated (for hospice patients only).
Communicates with community health related persons to coordinate the care
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician (or approved medical provider) and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group or team.
Assists the patient and family/caregiver and other team members in providing continuity of
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
Attends required staff meetings and interdisciplinary group meetings or case
Communicates report of services provided to the agency clinical leadership team and/ or RN Case Manager at the end of each shift.

Participates in on-call duties as defined by the on-call
Ensures that arrangements for equipment and other necessary items and services are
Coordinates patient care in tandem with other clinical staff as assigned by Clinical
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Actively participates in quality assessment performance improvement teams and activities or other committees as assigned.
May be required to work outside of designated geographical area based on census fluctuations and current staffing.
Completes mandatory education requirements annually to maintain
May be required to visit patients in homes that are in less than ideal
Driving conditions may be difficult related to weather and/ or
Maintains knowledge of and adheres to all applicable laws, rules, and
Participates in the orientation of new employees and preceptorship of students as
Meets productivity
Qualifications/Skillsets:
Graduate of an accredited school of nursing
One to two years of recent acute care experience in an institutional setting
One to two years of recent experience in hospice or home health preferred
Currently licensed as a Registered Nurse in the State(s) of planned practice
Possesses and maintains CPR certification
Communicate effectively, both written and orally, with all levels of the organization
Exceptional interpersonal skills both over the phone and in person
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Self-directed and able to work with minimal supervision
Capable of prolonged or considerable walking or Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Capable of considerable reaching, stooping bending, kneeling, and/or crouching. Possesses visual acuity and hearing to perform required nursing skills.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
